The usage of ID cards is not required to access care; employees can provide their social security number to their provider for benefits to be verified by the provider.

Please note that you may only use the contact lens allowance or the frame allowance, but not both within the same benefit period. Service frequencies are computed by date of service, NOT calendar year.

Vision Insurance Explained

Vision insurance is a specialized insurance plan that focuses on eye health and vision needs.

Most plans will cover routine eye exams and assist with the purchase of corrective vision products such as glasses and contacts.

Vision plans do much more than just pay for glasses; they help to preserve overall eye health. A routine eye exam looks at total eye health and includes a plan to help your eyes stay healthy.
