Making well-informed decisions about your benefits is an important part of being a consumer within a challenging health care system. It’s also increasingly important to select the right benefits to meet your family’s needs and effectively manage health care services and costs.

Annual Open Enrollment is a period of time when you can review, select, and make changes to your benefits. The purpose is to give you the opportunity to choose the benefits that best suit your needs and make any necessary adjustment for the upcoming year.

Who Is Eligible For Benefits?

You are eligible for benefits if you are a regular, full-time employee working an average of 30 hours per week.

Eligible Dependents include:

  • Your legal spouse or domestic partner
  • Children under the age of 26, regardless of student status, dependency, or marital status
  • Children who are fully dependent on you for support due to a mental or physical disability and who are indicated as such on your federal tax return; coverage may continue past age 26

New Hires:

To enroll in benefits, go to [Platform]. You will be required to register and create a log in and password. The system will prompt you to step through the process, from beginning of enrollment, updating personal information, dependent information and choosing benefits. Enrollment must be completed within xx days of date of hire.

Your elected coverage is effective the first of the month following date of hire.

Dependents:

The cost to you for dependent coverage will vary depending on the number of dependents you enroll in the plan and the particular plans you choose. When covering dependents, you must select the same plans for your dependents as you select for yourself.

What's New in 20xx/20xx

You’ll notice several changes to our medical and prescription drug benefits for the upcoming plan year. This Summary of Material Modification ("SMM") will modify some of the information contained in the Summary Plan Description ("SPD") for the [Name of Welfare Benefit Plan] (the "Plan") that describes the Plan as of [Date].

Note: In the event of any discrepancy between this information and the SPD (Summary Plan Description), the provisions of the SPD will govern.

When Can I Make Changes?

Once you elect your benefit options, they will remain in effect for the entire plan year until the following Open Enrollment. You may only change coverage during the plan year if you have a Qualifying Life Event, and you must do so within 30 days of the event.

Qualifying Life Events include:

  • Marriage, divorce, legal separation or annulment
  • Birth, adoption or placement for adoption of an eligible child
  • Death of a spouse or child
  • Change in your spouse’s employment that affects benefits eligibility
  • Change in your child’s eligibility for benefits (reaching the age limit)
  • Change in residence that affects your eligibility for coverage
  • Significant change in coverage or cost in your, your spouse’s or child’s benefit plans
  • FMLA leave, COBRA event, Court Judgment, or Decree
  • Becoming eligible for Medicare or Medicaid
  • Receiving a Qualified Medical Child Support Order

If you have a Qualifying Life Event and want to request a mid-year change, you must notify Human Resources and complete your election changes within 30 days following the event. Please be prepared to provide documentation to support the Qualifying Life Event.
